SERVICES IN THE STATE OF NEW YORK
Service Coordination
Service Coordination provides primary assistance of the waiver
participants in:
- Developing a Service Plan
- Gathering the information needed for the applicant to be approved
as a waiver participant
- Securing a residence in the community that meets the individual's
welfare and safety needs
- Implementing of the Service Plan
- Reviewing the outcomes of the Service Plan
- Revising the Service plan
Throughout his/her involvement with the waiver participant,
the service coordinator will support and encourage the waiver
participant to increase his/her ability to problem solve, be
in control of life situations, and be as independent as possible.
This is balanced by the need to assure the waiver participant's
health, safety, well-being, and inclusion in the community.
The waiver participant is the primary decision maker regarding
the services and supports to be included in the Service Plan,
as well as who will provide those needed services and supports.
Following the approval of the Service Plan the service coordinator
will assist the waiver participant with implementation of the
plan. The implementation of each Service will be unique; roles
and responsibilities will be determined based on the needs and
abilities of the waiver participant. The service coordinator
is responsible for assuriong that there is adequate coordination,
appropriate communication, and maximum cooperation between all
sources of support and services for the waiver participant.
Independent Living Skills Training and Development
Independent Living Skills and Development are services individually
designed to improve the ability of the waiver participant to
live as independently as possible in the community. Independent
Living Skills Training and Development may be provided in waiver
participant's residence and in the community. Services may include
assessment, training, and supervision of, or assistance to,
an individual with self-care, medication management, task completion,
communication skills, interpersonal skills, socialization, sensory/motor
skills, mobility, community transportation skills, reduction/elimination
of maladaptive behaviors, problem-solving skills, money management,
and ability to maintain a household.
Home and Community Support Services
Home and Community Support Services are individually designed
support services essential for the waiver participant's health
and welfare. These services include assistance, training, supervision
with activities of daily living, such as bathing, personal hygiene,
dressing, meal preparation, eating, light and heavy household
tasks, laundry, transportation, shopping, as well as socialization
and supervision. These services may be provided in the waiver
participant's home and in the community.
